Equifax was forwarded an identity theft affidavit in which they had four days to remove, block it. They have done nothing but sent me a letter stating that they refused to block it. this was not sent in error, and a copy of my id and social security was sent along with it. 1. THE INFORMATION I PRESENTED TO YOU WAS NOT BLOCKED IN ERROR 2. THE INFORMATION I REQUESTED IS IN THE FORM OF AN AFFIDAVIT OF FACT 3. IF YOU BELIEVE I OBTAINED POSSESSION OF GOODS AND SERVICES, OR MONEY AS A RESULT OF THE TRANSACTIONS I NEED A SENIOR EXECUTIVE MEMBER OF THIS COMPANY TO PUT IT ON AN AFFIDAVIT SIGNED UNDER THE PENALTY OF PERJURY AND NOTARIZED.
