Account United Collectioin # XXXX I'm unware of these accounts and did not have the opportunity to dispute them. Please provide Validation of Debt, Assigment of Debt, and Method of Verification. Please provide the " certfied mail receipt '' showing I have recieved the notification, per the law. Please all request listed of you will be in violation of the FCRA. Retreat this account from collections and remove it from my credit report, if you have not already done so. According to HIPAA, you have no permissible business reason to supply a third party collection agency any information regarding myself or my medical history due to no money being owed. I have no desire to cause you unnecessary difficulty, however this unnecessary negative information reporting on my credit profile is beyond damaging. Once I receive the response from the credit reporting agencies and if the account HAS NOT been deleted, I will be forced to take action to protect my rights as a consumer under HIPAA, FCRA and FACTA rules and regulations. Rather than through what would undoubtedly be a hassle for both you and I, please do as you have been instructed and what is right according to HIPAA requirements. While, I would rather handle this between you and myself, I am prepared to do what needs to be done to protect my rights.
