on or about XX/XX/2021, I received a notice from Credit Collection Services that I owed XXXX {$330.00}. I called CCS and advised them to send me proof of services and debt. CCS referred me to XXXX and I explained to CCS that they need to send me the proof of services and debt if they are collecting on XXXX 's behalf. On or about XX/XX/2021 XXXX sent me notice that they will report me to all three credit bureau if I don't pay by XX/XX/2021. I called XXXX and advised them that I have not received any documentation from them on the said debt and or services from XXXX. They told me that they would send me the Health Insurance Claim form. I did receive the Health Insurance claim from which had my name and date of birth, however, it had a bad address and doctor that I had never seen or heard of. The Claim form had 'signature on file ' and no verification or actual signature. I requested that CCS go back and send me proof of debt/ service with my signature authorization such as service and doctor. CCS did not send me proof and reported me to all three credit bureaus.
