I was a patient at a XXXX XXXX treatment facility called XXXX XXXX, XXXX XXXX from XX/XX/2018 until XX/XX/2018. I was initially admitted to the facility under the agreement that the cost of my stay would be covered by my health insurance, XXXX XXXX XXXX. After six weeks into my stay, it was discovered that my health insurance had terminated due to a payment issue with the cobra website and I was informed I had to sign an agreement that stated I would be responsible for the total cost of my stay at inpatient and the remainder of my stay at their XXXX XXXX and XXXX. ( My health insurance had mistakenly approved all the days of my stay up until that point ) I was told that if I did not sign this agreement I would have to leave the facility granted at the time I was still medically recovering and did not have any where else to go. I reluctantly signed the payment responsibility agreement under duress and informed the staff at the facility that I would work with my former employer and cobra to try to have my health insurance reinstated so the payment matter would be fixed. Two to three weeks later while I was waiting to hear back from my former employer, the staff at the XXXX XXXX XXXX XXXX XXXX facility told me they could no longer wait for payment and that I had to be discharged. The director of their XXXX facility, XXXX XXXX and her coworker were extremely bullying and stated to me this is your fault because of your XXXX XXXX XXXX XXXX Three months later, on XX/XX/2018, I received a letter that stated I owed a {$13000.00} + bill to XXXX XXXX for the cost of my stay at their facilities. I called their billing department to inform them that I was not responsible for the payment and that the responsibility belonged to my former health insurance provider, XXXX XXXX XXXX. The bill was eventually turned over to a creditor, Credit Management. The amount for the bill now stands at {$11000.00} I do not owe this money, I signed responsibility for the payment only because I was under duress and I was not mentally/medically in the right state of mind to sign as evidenced by my then need for a stay at a treatment center. My signature on the patient financial responsibility document ( attached ) was not notarized.
