I am contacting you to dispute the charges being claimed on my account. I had an insurance policy with your company for XXXX month. Before the first month ended, I cancelled the policy and clearly stated that I did not want to continue into a second month. I did not use or authorize any coverage for the second month. Despite this, I was billed and the amount was sent to collections. This charge is incorrect and account is closed, they provided me instruction to close the account. That's unfair show the status of account open. I am requesting : Verification of the debt, including proof that I authorized or used coverage for the second month. Immediate correction of my account and removal of the collection notice. Written confirmation once the issue is resolved.
