FORMAL BILLING DISPUTE & FINAL DEMAND FOR REFUND Date : XX/XX/XXXXXXXX To : Affirm , Inc. / XXXX XXXX XXXX XXXX XXXX. RE : Order # XXXX | Affirm Case # XXXX | XXXX Ref # XXXX To Whom It May Concern, I am filing this formal dispute regarding an uncredited balance of {$620.00} on my account. Despite providing proof of returns multiple times since XX/XX/XXXX, Affirm and XXXX XXXX ( XXXX ) have failed to reconcile my account, in violation of the Fair Credit Billing Act ( FCBA ). 1. Proof of Return & Receipt : I have provided XXXX tracking numbers XXXX ( Delivered XX/XX/XXXXXXXX ) and XXXX ( Delivered XX/XX/XXXX ). These confirm the merchant received the full return packages. 2. Merchant Admission of Error : On XX/XX/XXXX, XXXX XXXX ( Ref # XXXX ) confirmed that my returned items were received at the warehouse but were never reported to Affirm. The merchant admitted this was due to flagging the account for the " large volume '' of the return. 3. Lender ( Affirm ) Admission of Accounting Error : On XX/XX/XXXX, an Affirm employee verbally admitted to me that Affirm has " XXXX XXXXp '' my loan amounts and that the accounting on this specific balance is incorrect. Despite this internal admission of a billing and accounting error, Affirm has failed to update the balance, has continued to charge interest, and has repeatedly closed my dispute cases without resolution. 4. Legal Violations and Demand : My formal notice of dispute was sent on XX/XX/XXXX. Under the FCBA, Affirm had 90 days to resolve this. By failing to correct an admitted accounting error and ignoring proof of delivery for over seven months, Affirm is in willful violation of federal law. I have made {$360.00} in overpayments since XXXX to protect my credit while Affirm failed its legal obligations. There were 30 items total out of those XXXX XXXX XXXX send four items back to me saying that I couldnt return them. Another item was never sent to me at all and I was charged and they only reported eight items returned making it 18 items that they did not report that were returned that I have emails for each item was returned, but they failed to report that to Affirm or confirm it every time I filed a case disputing this. Today I spent an hour talking to an XXXX XXXX employee, giving them each item that was returned each item that XXXX XXXX returned to me and the item that was never sent to me, but I was charged for. He said that he started a new dispute, but I never got any kind of confirmation from that dispute. All I have is a confirmation from a firm also who I spoke to today who said that XXXX XXXX really messed up onto two of my orders and returned money to one order and not the other, which messed everything up. I demand the following immediately : Full credit of the remaining {$620.00} principal. A cash refund of the {$360.00} in overpayments. A full refund of all interest and finance charges since XX/XX/XXXX. Immediate correction of my account to reflect a {$0.00} balance. Sincerely, XXXX XXXX ( XXXX ) XXXX
