Accounts in Question : Bridgecrest Account # XXXX ( Opened XX/XX/XXXX ) Bridgecrest Account # XXXX ( Opened XX/XX/XXXX ) Dear Bridgecrest XXXX Department, I am writing to formally notify you that the above-referenced accounts were fraudulently opened in my name without my knowledge or authorization. I am a victim of identity theft, and I have filed a police report documenting this crime. Copies of my police report, government-issued identification, and proof of address are enclosed for verification. Under the Fair Credit Reporting Act ( FCRA ), 15 U.S.C. 1681c2 ( a ), once a consumer provides an identity theft report, a creditor or furnisher is required to block and delete all information resulting from identity theft from the consumers file.
