Formal Dispute of Inaccurate Information Under FCRA Identity Theft To Whom It May Concern, I am writing to formally dispute inaccurate information that Early Warning Services ( XXXX ) has reported about me. I am a victim of identity theft, and the negative information appearing on my XXXX XXXX consumer report is not mine and was caused by a fraudulent individual. In XXXX, I disputed a fraudulent XXXX XXXX XXXX account that appeared on my Early Warning Report. The report showed that the person who opened the account used a Texas drivers license, but at that time I had an XXXX drivers license, proving it could not have been me. I provided a police report and documentation showing that the account was opened by an identity thief, not me. After my dispute in XXXXXXXX XXXX XXXX XXXXXXXX removed the fraudulent information from my report. However, I recently discovered that XXXX XXXX reinserted the same inaccurate XXXX XXXX XXXX information without notifying me or completing a reinvestigation as required by law. XXXX XXXX also failed to provide me with any documentation showing certification or proof that the information was verified. I am requesting the following : Immediate deletion of the XXXX XXXX XXXX information that was previously removed. A full reinvestigation, including verification from XXXX XXXX XXXX A copy of all documents XXXX used to verify this information. Written confirmation that the inaccurate information has been permanently removed from my consumer file. Please consider this letter a formal dispute under the Fair Credit Reporting Act. If this matter is not corrected promptly, I will pursue further action through the CFPB and consider additional legal remedies. Furthermore, although XXXX XXXX XXXX states in the attached correspondence that they do not have access to account documentation and can not provide copies of any records, XXXX XXXX previously provided me with a signature form allegedly from XXXX XXXX XXXXXXXX. I have documentation proving that XXXX XXXX supplied this form to me. The signature on that document is not my signature, and I am formally disputing it as fraudulent and inaccurate. This inconsistencyclaiming they can not access account documents while providing me with a signature XXXX procedural errors in their reporting process. I am requesting that this inaccurate and unverified information be immediately removed from my consumer report in compliance with the Fair Credit Reporting Act ( FCRA ).
