Original Creditor : XXXX XXXX Date Opened : XX/XX/year> Balance : {$4100.00} Last Reported : XX/XX/year> Status : Derogatory / Placed for Collection Account # : XXXX XXXX XXXX XXXX XXXX I dispute this debt in its entirety.
