EVEN AFTER NOTICE OF DISPUTE, THE AGENCY CONTINUES TO REPORT THE SAME ERRONEOUS INFORMATION TO THIRD PARTIES AND AFFILIATES, KNOWING THAT IT LACKS DOCUMENTED PROOF OR VERIFIABLE SOURCE RECORDS.
