I am writing to formally notify you that I am a victim of a data breach and identity theft. As such, I am requesting, pursuant to 15 U.S.C 1681c-2 ( a ) of the Fair Credit Reporting Act, that all accounts and information associated with fraudulent activity in my name be permanently deleted and blocked from your records.
