XXXX XXXX XXXX XXXX XXXX * * * * XXXX RE : Identity Theft Dispute Request to block XXXX XXXX XXXX XXXX XXXX * * * * XXXX XXXX To Whom It May Concern, I am writing to dispute and request the immediate blocking and removal of a fraudulent account listed on my credit report in accordance with Section 605B of the Fair Credit Reporting Act ( FCRA ).
