XXXX XXXX XXXX, GA XXXX Subject : Request for Removal of Unauthorized XXXX Inquiries under the Fair Credit Reporting Act ( FCRA 604, 611 ) To Whom It May Concern : I am writing to formally dispute and request the immediate removal of multiple unauthorized XXXX inquiries appearing on my Equifax credit report.
