I am submitting this new complaint regarding a previously disputed medical debt ( Account # # # # # # # # XXXX ) for {$730.00}, originally submitted to the CFPB in XX/XX/XXXX. Although my original complaint was closed, the company waited over XXXX months ( until XX/XX/XXXX ) to respond and is now making new claims that I never had an opportunity to address during the initial investigation. Specifically, the company now claims that they sent me a letter regarding the alleged debt, but I have never received any such communication, nor any : Itemized statement Explanation of charges Evidence of services rendered Documentation of billing to my insurance ( XXXX ) The alleged date of service is XX/XX/XXXX, during which time I had active health insurance coverage through XXXX. I continue to dispute the validity of this debt and have not been provided with any verifiable evidence that services were rendered to me or that proper billing procedures were followed. Because the companys delayed response came after the complaint was closed, I am requesting that the CFPB : Reopen or re-investigate this matter Require the company to provide documentation of the debt and proof of any letter they claim to have sent ( with dates, address, and delivery method ) Ensure this account is not reported to credit bureaus while under dispute I am prepared to provide documentation of my insurance coverage at the time and have made every effort to resolve this matter in good faith.
