I am submitting this formal complaint against Equifax for multiple violations of the Fair Credit Reporting Act ( FCRA ) in connection with fraudulent accounts that remain on my credit report despite my repeated disputes and submission of legally sufficient documentation. The account in questionXXXX XXXXXXXX, was opened fraudulently and are the result of identity theft. I provided Equifax with a government-issued ID, proof of address, an FTC Identity Theft Report, and a signed statement under penalty of perjury, all of which satisfy the requirements under FCRA 605B . Despite this, Equifax failed to block the fraudulent information within four business days, as required by law. Furthermore, Equifax has violated FCRA 611 ( a ) by failing to conduct a proper reinvestigation of my dispute within the 30-day statutory window. I also requested full disclosure of all information used to verify the disputed account, as permitted under FCRA 609 ( a ) ( 1 ) ( A ), but Equifax has not provided this information. These failures constitute both negligent and willful noncompliance with the FCRA, entitling me to damages under 616 and 617.
