Consumer Narrative

Dear Sir or Madam, I am writing to formally dispute multiple accounts currently listed on my credit file. Under the Fair Credit Reporting Act ( FCRA ), Fair Debt Collection Practices Act ( FDCPA ), and, for medical accounts, the Health Insurance Portability and Accountability Act ( HIPAA ), I demand immediate reinvestigation and removal of any unverifiable or inaccurate information. FCRA - 15 U.S.C. 1681e ( b ) : Requires credit reporting agencies to ensure maximum possible accuracy of the information they report. - FCRA - 15 U.S.C. 1681i ( a ) : Requires a reasonable reinvestigation of disputed information within 30 days. - FCRA - 15 U.S.C. 1681c-2 : Governs reporting of medical debts, requiring accuracy and compliance with time limits. - FDCPA - 15 U.S.C. 1692g : Grants consumers the right to request validation of debts. - FDCPA - 15 U.S.C. 1692e ( 8 ) : Prohibits communicating false or misleading information to credit reporting agencies. - HIPAA - 45 CFR 164.502 & 164.508 : Prohibits unauthorized disclosure of protected health information without written consent. My Demands : 1. Provide full validation for each account, including copies of any original signed contracts or agreements proving I am liable. 2. For medical accounts, provide proof of HIPAA-compliant authorization allowing the disclosure of my information to you. 3. If you can not validate the accuracy and legality of any account, delete it immediately from my credit file. 4. Provide me with the name and contact information of the original creditor and any third-party furnishers of this data. 5. Send me written confirmation of the results of your reinvestigation within the 30-day period required by law. Notice of Intent : If these accounts are not removed or corrected within 30 days, I will file complaints with the Page 2 Consumer Financial Protection Bureau ( CFPB ), Federal Trade Commission ( FTC ), U.S. Department of Health and Human Services ( for HIPAA violations ), and my state Attorney General 's office. I reserve the right to seek all remedies available under the law for willful noncompliance.
