XXXX XXXX, XXXX, PA XXXX Subject : Formal Dispute Fraudulent Accounts Resulting from Identity Theft XXXX XXXX XXXX XXXX ( {$1900.00} ) XXXX XXXXXXXX XXXX XXXX ( {$420.00} ) To Whom It May Concern, I am a confirmed victim of identity theft, with my personal information compromised in both the XXXX XXXX XXXX XXXX and the Equifax Data Breach. The following provisions specifically apply to my dispute : 15 U.S.C. 1681c-2 ( FCRA 605B ) Requires blocking and permanent deletion of information resulting from identity theft upon receipt of an FTC Identity Theft Report. 15 U.S.C. 1681i Mandates reasonable reinvestigation of disputed information and deletion if unverifiable. 15 U.S.C. 1681e ( b ) Requires maximum possible accuracy in the reporting of consumer information. 15 U.S.C. 1681b Limits permissible purposes for furnishing a consumer report. 15 U.S.C. 1681n and 1681o Provides for liability for willful and negligent noncompliance. 15 U.S.C. 1681q Prohibits obtaining or using consumer information under false pretenses.
