: Unauthorized Hard Inquiries on My Credit Report Report Date : [ Insert Report Date ] SSN ( last 4 digits ) : XXX-XX- [ # # # # ] To Whom It May Concern : I am writing to formally dispute unauthorized hard inquiries that appear on my credit report maintained by your bureau. I am also prepared to pursue legal remedies for willful noncompliance, which may include statutory damages under FCRA 616 and 617.
