Equifax, I am a victim of identity theft. The information listed below, which appears on my credit report, does not relate to any transaction that I have made. My personal information has been compromised through Equifax data breach, XXXX data breach and XXXX XXXX data breach. Please remove and block this information from my credit report, pursuant to section 605B of the Fair Credit Reporting Act, and send the required notifications to all furnishers of this information. The fraudulent accounts are as follows : XXXX XXXX {$1200.00}. XXXX XXXX {$6300.00}. XXXX XXXX XXXX {$1100.00}. XXXXXXXX XXXX XXXX is not mine. Section 605B of the Fair Credit Reporting Act, REQUIRES you to block the fraudulent information on my credit report resulting from identity theft within four business days and to promptly notify the furnisher ( s ) of that information. 15 USC 1681i requires 100 % accuracy of all reported information. Additionally, Equifax has allowed re- insertion of some items to my credit report. This is a request for your method of verification under Section 611 of the FCRA. Equifax 's response should include the documents used to confirm the accuracy of all " verified and or re-inserted tradelines reported tradelines. This includes but is not limited to XXXX XXXX, XXXX XXXX XXXX, XXXXXXXX XXXX Original credit application, account statements, or accurate identification.
