This letter serves as my FOURTH FORMAL NOTICE regarding inaccurate personal information appearing in my consumer report maintained by youre a organization. I have previously submitted XXXX notices. Despite my repeated requests, the inaccurate inf " ormation remains uncorrected. This constitutes a continued violation of the Fair Credit Reporting Act ( 15 U.S.C. 1681 et seq. ), which requires consumer reporting agencies and furnishers to conduct a reasonable reinvestigation when a dispute is submitted by a consumer.
