To Whom It May Concern, I am writing to formally dispute and request the removal of all negative items that appear on my credit report as a result of identity theft, in accordance with 15 U.S.C. 1681c-2, which states that a consumer reporting agency must block the reporting of information that the consumer identifies as resulting from identity theft. I believe the following negative items do not belong to me and are the result of fraudulent activity or misreporting : All negative entries, including collections, charge-offs, late payments, and accounts not initiated or authorized by me. Under 15 U.S.C. 1681c-2, I am exercising my right to have these items blocked from my credit report. Attached you will find the following documentation as required under this law : A copy of a valid government-issued photo ID Proof of residence ( utility bill, lease, or bank statement ) A copy of a police report or identity theft report filed with a law enforcement agency or the FTC Please conduct an investigation and block the reporting of the aforementioned items as mandated by federal law.
