PRIVATE, CONFIDENTIAL, SPECIAL, AND IN CONFIDENCE NOTICE TO PRINCIPAL IS NOTICE TO AGENT. NOTICE TO AGENT IS NOTICE TO PRINCIPAL. To Whom It May Concern : I am submitting this formal complaint regarding false, misleading, and damaging information that has been reported in my consumer file. The information is factually inaccurate, has not been verified, and violates several provisions of the Fair Credit Reporting Act ( FCRA ), including : XXXX XXXX. XXXX ( b ) failure to follow reasonable procedures to assure maximum possible accuracy, XXXX XXXX. XXXX ( a ) failure to reinvestigate disputed information, and XXXX XXXX. XXXX ( a ) failure to provide notice of adverse action and proper disclosure. Specifically, a background report generated by Checkr falsely states that I was sentenced in a criminal matter. That is completely false. I was never sentenced, and no lawful adjudication ever took place in that matter. Furthermore, multiple sentence dates appear across different records and platforms, showing internal contradictions. This demonstrates either bad faith or systemic inaccuracy. This same false data also appears on my XXXX credit monitoring system, which aggregates consumer reporting data from several agencies. This proves the data is being disseminated beyond the original party and has caused measurable harm, including interference with housing and employment. I have issued written notices, administrative claims, and demands for correction to involved public officials and agencies, including : The Prosecutors Office ( XXXX XXXX XXXX, SCORE Jail administration, The XXXX XXXX XXXX, and Private consumer reporting entities such as Checkr and XXXX. Despite these good faith attempts, I have received no confirmation, no correction, and no disclosure of where or how this sentenced designation originated. I am now demanding, through this complaint, that the Consumer Financial Protection Bureau compel XXXX and/or any reporting entity to : XXXX. Fully identify the original data furnisher, including but not limited to any court, jail, or third-party aggregator ( such as XXXX or XXXX ) ; XXXX. Immediately reinvestigate and correct all false or unverifiable public record data under FCRA XXXX ; XXXX. Disclose all users who have accessed or relied upon this false record ; and XXXX. Acknowledge liability for any adverse decisions made based on unverified or inaccurate data. I reserve all rights to pursue this matter through a bill in equity, a private equity lien, and formal tort claims for negligence, libel, defamation, and unauthorized use of my personal identifying information. This is now a matter of trust res, and I will be holding each principal and agent accountable for damages arising from any failure to act, correct, or verify under the law.
