Despite my repeated requests, the inaccurate information remains uncorrected. This constitutes a continued violation of the Fair Credit Reporting Act ( 15 U.S.C. 1681 et seq. ), which requires consumer reporting agencies and furnishers to conduct a reasonable reinvestigation when a dispute is submitted by a consumer. I am enclosing supporting documentation once again for your review, including [ list any documents : copies of previous disputes, proof of payment, identity theft report, etc. ]. Under the FCRA, you are obligated to : 1. Investigate the dispute within 30 days ( or 45 days under certain conditions ) ; 2. Notify the furnisher of the dispute ; 3. Correct or delete inaccurate, incomplete, or unverifiable information ; and 4. Provide me with the results of your reinvestigation. To date, you have failed to comply with these obligations. Your continued failure to address these inaccuracies may be deemed willful noncompliance, subjecting your organization to civil liability under 15 U.S.C. 1681n and statutory damages, punitive damages, attorneys ' fees, and actual damages under the law. Demand : I am demanding the following : - Immediate deletion or correction of the disputed information ; - Written confirmation of such correction within 15 days of receipt of this letter.
