Subject : Request to Block Fraudulent Accounts Pursuant to 15 U.S.C. 1681c-2 I, XXXX XXXX XXXX XXXX XXXX am formally submitting this statement to dispute fraudulent information included in my credit report, pursuant to my rights as a consumer under the Fair Credit Reporting Act ( FCRA ), specifically 15 U.S. Code 1681c-2.
