I am a victim of identity theft and have provided all the required documentation under FCRA 605B to the credit bureau, including : A copy of a valid government-issued ID ; A copy of a police report or FTC Identity Theft Report documenting the identity theft ; A written statement identifying the specific fraudulent items and requesting the blocking of such information. Despite providing these documents and making the request on [ insert date ], the credit bureau has failed to block the disputed information within the required 4-business-day window or has reinserted the information without providing proper notice, which violates FCRA Section 605B. As a result, the fraudulent information remains on my credit report and is negatively affecting my creditworthiness, including being denied credit and/or receiving unfavorable terms due to the presence of accounts or inquiries that are not mine. I am requesting that the CFPB take action to ensure the credit reporting agency complies with its legal obligations under FCRA 605B and immediately and permanently block the fraudulent information from my credit report.
