To Whom It May Concern, I am formally disputing the inclusion of certain accounts on my consumer credit report that have been reported without my explicit consent, in violation of federal law. Pursue punitive damages for the reckless and harmful reporting of unverified and unauthorized information. This is your final notice. I expect full compliance within the legally mandated timeframe of 30 days from receipt of this letter.
