XXXX XXXX XXXX, GA XXXX Subject : Request to Block and Delete Fraudulent Accounts Resulting from Identity Theft Due to Equifax XXXX XXXX Pursuant to 15 U.S.C. 1681c-2 ( a ) To Whom It May Concern : I am writing to formally notify Equifax that XXXX am a victim of identity theft, which I believe is a direct result of the Equifax XXXX XXXX that occurred in 2017. As such, I am requesting that, pursuant to 15 U.S.C. 1681c-2 ( a ) of the Fair Credit Reporting Act ( FCRA ), all accounts and related information resulting from this identity theft be blocked and permanently deleted from my credit file. This breach has compromised my personal information, and fraudulent accounts have since been opened in my name without my knowledge or authorization. I am exercising my rights under federal law to dispute and remove this inaccurate information from my credit report. Enclosed are the following documents to support my request : A copy of my government-issued photo identification ; Proof of my current mailing address ( e.g., utility bill or bank statement ) ; A copy of the police report or Federal Trade Commission Identity Theft Report documenting the identity theft ; A written statement listing the accounts I dispute and request to be blocked and deleted. Please investigate and take action to block and delete all fraudulent accounts from my credit report as required by law. I also request written confirmation of the results of your investigation and the actions taken. Thank you for your prompt attention to this matter.
