Subject : Request pursuant to FCRA 605B ( 15 U.S.C. 1681c-2 ) all accounts fraudulent in this complaint be DELETED immediately. I am submitting this complaint to request immediate intervention due to serious violations of the Fair Credit Reporting Act ( FCRA ), Fair Debt Collection Practices Act ( FDCPA ), and Truth in Lending Act ( TILA ) by EQUIFAX, Despite previous requests for correction, these violations continue to negatively impact my credit report and financial standing. I am writing to contest inaccurate information on my credit report in accordance with 15 U.S.C. 1681c-2 and to formally request the blocking of this erroneous data. Enclosed is a copy of my Police Report/FTC identity theft report ( FTC REPORT XXXX : XXXX ) as evidence supporting my identity theft claim. As per 15 U.S.C. 1681c-2, upon submission of an identity theft report by a consumer, credit reporting agencies are obligated to block any information flagged by the consumer as stemming from identity theft within XXXX business days. The following information on my credit report is the result of identity theft and should be blocked. I request, pursuant to FCRA 605B ( 15 U.S.C. 1681c-2 ), that you, the credit reporting agency EQUIFAX, block the information appearing in my consumer credit report resulting from identity theft and fraud within 4 business days of receiving this complaint. PLEASE DELETE THIS ACCOUNT IMMEDIATELY. I do not recognize the accounts mentioned as reported. The accounts mentioned above in my consumer credit report that you maintain were not opened, created, or initiated by me. The accounts mentioned above are the result of identity theft and fraud. I did not authorize the sharing of my information nor did I consent to its reporting. I was not provided with adequate information or the option to opt out. Reporting without consent constitutes fraud for the purposes permitted by Title 15, United States Code ( USC ), Section 1981b2. This also violates my privacy rights under Title 15, USC, Section 1681a ( 4 ). These accounts must be immediately blocked or removed from my consumer report, pursuant to FCRA 605b.
