I, XXXX XXXX XXXX, hereby state that I have become a victim of identity theft. Unauthorized parties have used my personal information without my permission. Evidence of this theft has already surfaced : I have received debt collection calls and see entries on my credit report for accounts that I never opened or authorized. In light of these developments, I am taking formal action to resolve the fraud. I believe the source of this theft was the XXXX Equifax data breach. In XX/XX/year>, Equifax announced that a breach exposed the personal information of approximately XXXX XXXX Americans. Because my data was likely compromised in that incident, it is probable that the fraudulent accounts now appearing on my reports stem from that breach. Specifically, the following accounts have appeared on my credit reports without my knowledge or consent : XXXXXXXX XXXX XXXX XXXX XXXX XXXX XXXX XXXX XXXX XXXX XXXX XXXX XXXX XXXX XXXX XXXX I affirm that I did not open or authorize any of the above accounts, and I dispute all debts associated with them. These accounts and debts are fraudulent and are not mine.
