XXXX XXXX XXXX, GA XXXX Subject : Fraudulent Account Notification Request for Immediate Block Under FCRA 605B Dear Sir or Madam, I am writing to formally notify you that an account listed on my credit report under the name XXXXXXXX XXXX XXXXXXXX XXXX is fraudulent. I did not open or authorize this account, and I believe it was opened as a result of identity theft.
