I am writing to formally request the immediate removal of fraudulent and unauthorized accounts from my credit report under the Fair Credit Reporting Act ( FCRA ), 15 U.S. Code 1681c-2 ( 605B ). The following accounts were opened or reported without my knowledge, authorization, or consent and are the result of identity theft. Under FCRA 605B, I have the legal right to request that any consumer reporting agency block the reporting of information that I identify as resulting from identity theft.
