As a result, there are fraudulent accounts and/or inquiries on my credit report that I did not authorize or open. Formal Request for Investigation and Debt Verification : Under FCRA 605B ( 15 U.S.C. 1681c-2 ), I am requesting that the credit reporting agencies immediately block and delete the above-mentioned fraudulent accounts from my report. Under FCRA 611 ( 15 U.S.C. 1681i ), I am also requesting full debt verification. I demand that the credit bureaus and the original furnishers provide valid proof including any signed applications, contracts, or identification that demonstrate I personally authorized these accounts. If such verification can not be provided, the accounts must be permanently removed from my credit file. Compensation for False Verification : Under FCRA 616 ( 15 U.S.C. 1681n ), if any of these accounts are falsely verified as valid despite my claim of identity theft, I reserve the right to pursue statutory damages of {$1000.00} per violation.
