Complaint against : XXXX, TransUnion, XXXX, and XXXX XXXX XXXX : Name : XXXX XXXX XXXX XXXX Address : XXXXXXXX XXXX XXXX XXXX XXXX XXXX XXXX XXXX XXXX XXXXXXXX I am filing this complaint against XXXX, TransUnion, XXXX, and XXXX regarding inaccurate and incomplete information on my consumer report. Pursuant to my rights under the Fair Credit Reporting Act ( FCRA ), 15 U.S.C. 1681 et seq., I have sent ( and attached to this complaint ) a formal letter demanding the following actions from this company : 1. Dispute of Entire File : I dispute the accuracy and completeness of all information contained in my consumer report held by this company, including personal data, accounts, public records, inquiries, and sources. 2. Full Reinvestigation : I demand a thorough reinvestigation of every item within the FCRA-mandated 30-day timeframe. I do not consent to any extensions. If the investigation is not completed within 30 days, the disputed information must be deleted. 3. Demand for Original Contracts : I demand copies of the original, handwritten, ink-signed contracts or agreements for every account listed on my report furnished to this agency. If unavailable, provide all documentation establishing the debt and terms. 4. Demand for Full Credit Report : I demand a complete copy of my consumer file disclosure ( full credit report ) from this agency. 5.
