To Whom It May Concern, I am writing to formally dispute inaccurate, unauthorized, and negative information that appears on my credit report. I request that the following items be investigated and removed or corrected immediately as required by the Fair Credit Reporting Act ( FCRA ). The disputed items include : 1. Unauthorized/Inaccurate Inquiries There are several inquiries listed on my report that I do not recognize, did not authorize, and have no affiliation with. These inquiries are affecting my credit score and do not reflect my credit activity. 2. Unknown Accounts There are accounts showing on my credit report that I never opened. I believe these to be the result of identity theft or reporting errors. These accounts are negatively affecting my credit profile. 3. Negative Information I also dispute any and all negative items that are inaccurate, unverifiable, outdated, or not in compliance with federal credit reporting regulations. Please investigate these entries and delete any that can not be verified.
