I, XXXX XXXX, am a victim of identity theft. Under FCRA 605B, 615 ( f ), and 6023 ( a ) ( 6 ), an identity theft report can be used to permanently block fraudulent information from appearing on my credit profile, including unauthorized accounts or addresses. This also ensures that such debts do not reappear on my credit report. Additionally, an identity theft report prevents companies from continuing to collect debts that result from identity theft. A copy of the relevant FCRA sections is enclosed. Under FCRA 605B ( 15 U.S.C. 1681c-2 ), consumer reporting agencies must block the reporting of any fraudulent information within four ( 4 ) days after receiving the following : Proof of identity, including copies of my Drivers License and Social Security Card ( provided ). A copy of my official Identity Theft Report. Identification of fraudulent information, including a list of accounts opened as a result of identity theft. A completed FTC Identity Theft Affidavit and Complaint ( attached ). I am disputing the XXXX account ( Account XXXX : XXXX ), which was fraudulently opened in my name. I request that you comply with federal law and immediately and permanently block this account, along with any related addresses, inquiries, or other fraudulent information from my credit report, as required under FCRA 605B, 615 ( f ), and 6023 ( a ) ( 6 ).
