Subject : Dispute of Unauthorized Hard Inquiries on My Credit Report ( Identity Theft ) Dear Credit Bureau Dispute Department, I recently reviewed my credit report and discovered several unauthorized hard inquiries that I did not initiate. These inquiries were made without my consent and are a direct result of identity theft. Per the Fair Credit Reporting Act ( FCRA ) 604, hard inquiries require my explicit authorization.
