I am formally disputing inaccurate and fraudulent information on my Early Warning Services ( EWS ) report. The following accounts do not belong to me and appear to be the result of identity theft or a reporting error : * XXXX XXXX Account # XXXX * XXXX XXXX Account # XXXX * XXXX XXXX XXXX XXXX Account # XXXX I demand an immediate investigation and the removal of these accounts from my record under my rights granted by 15 U.S. Code 1681i ( Procedure in case of disputed accuracy ) of the Fair Credit Reporting Act ( FCRA ). Additionally, failure to correct these inaccuracies may constitute violations of : * 15 U.S. Code 1681e ( b ) Failure to maintain accurate reporting procedures * 15 U.S. Code 1681s-2 Reporting false information with knowledge of errors * 15 U.S. Code 1681c-2 Blocking information resulting from identity theft Enclosed are supporting documents, including : * A copy of my government-issued ID * My EWS report * A signed fraud affidavit I am also attaching the FTC identity theft report related to this dispute. Under 15 U.S. Code 1681i ( a ) ( 1 ) ( A ), you are required to investigate and correct errors within 30 days. Please confirm receipt and provide written confirmation upon resolution.
