Subject : Fraudulent Account Dispute - ID Theft Case for Account ( Capital One Bank USA, N.A. ) To Whom It May Concern : I am writing to formally dispute the above-referenced account, which has erroneously reappeared on my credit report despite prior evidence submitted confirming it as fraudulent. Background : I became a victim of identity theft. The individual responsible unlawfully opened a Capital One account in my name without my authorization. I provided a comprehensive report to Capital One and all major credit reporting agencies, including : 1. An identity theft report from the Federal Trade Commission ( FTC ), 2. A police report. 3.
