XXXX XXXX XXXX XXXX XXXX XXXX XXXX XXXX XXXX XXXX XXXX I am requesting that you please provide me with the following information as proof that these items were reinserted in accordance with the law and if not please delete them from my consumer file immediately. 1. Proof that the furnisher certified that this information was accurate and complete. 2. Proof that you notified me, in writing, no later than 5 business days after the Reinsertion. 3. Proof that you provided me, in writing or no later than 5 business days after the date of Reinsertion. 4. The business name and address of any furnisher of information. Please provide me with documents and not a automatic dispute response.
