Customer name XXXX XXXX : Account number : XXXX To whom it may concern, I recently received a dispute resolution from a fraud dispute that was filed on XXXX XXXX. The resolution decision indicated that I was held liable for the fraud charges that occurred on my initial account XXXX XXXX XXXX XXXX because the card was activated using my home cell which I completely disagree with. I contacted your financial institution because I was a victim of fraud and that my card had been compromised, as a result of me being a victim of fraud, where my card was compromised and intercepted. I reached out to your Representative of XXXX XXXX and spoke with an advocate named XXXX a billing/ dispute XXXX who I informed that the decision was based off of ignorance falsely claiming the details revolving around the decision. I believe your lack of interpretation caused the denial of the decision. First to inform you I have never had a home phone in over a decade, secondly I didnt receive my replacement card until XXXX where i activated my replacement card from my mobile cell, I also concluded concrete evidence and proof to support my allegations on XXXX XXXX I submitted an email to XXXX pertaining a FTC Fair Trade Commission Report reporting the fraud that happened on my accounts and a Police Report from XXXX XXXX XXXX XXXX the proof the reference number for the docs is XXXX. I was also informed by me XXXX that those documents that you received on XX/XX/year>, was not attached to the disputed fraud claim nor were they factored in for the Decision that was made on XXXX XXXX. As a consumer who understand his rights and a consumer law XXXX who understands the FCRA and Congressional Laws, Im very disappointed in your judgment call and let down as a consumer, this has tarnished my credibility, has lowered my credit score, have affected my ability to obtain credit and has taken a severe toll on me mentally, emotionally, financially! With what you have allowed to furnisher as a data Furnisher! 15 US C 1681 a ) b ) Duty as a Data Furnisher is to provide exceptional information and accurate information to the CRA knowingly furnishings inaccurate information goes against the FCRA and I have put you on noticed for violating the FCRA Laws and regulations with what you have allowed to be reporting. I have attached the documents with this email and have also attached proof of the phone record from me speaking with your financial institution today XX/XX/year>. I will also be contacting the Consumer Financial Protection Bureau as well as the Ohio XXXX Commission to engage in rectifying this matter! I have put you on notice. You have XXXX days from the time this email was received to rectify this issue and appeal that this decision and make the proper adjustments, failure to comply with my request will result in me filing a suit against your financial institution seeking compensational damages.
