Enclosed are : my drivers license, my social security card, proof of my current address, my Federal Trade Commission Identity Theft Report, and a copy of SECTION 605B of the Fair Credit Reporting Act. I am writing to request that you remove/delete and place an immediate block on the fraudulent information listed below from my credit file within XXXX days upon receipt of this correspondence. I have recently obtained a copy of my credit file and the information listed below does not relate to any transactions that I have made ; therefore I am requesting that these items be completely removed/deleted and blocked from my credit file. This is not a regular dispute. I have filed a complaint with the Federal Trade Commission to document being a victim of identity theft.
