As a result of this breach, I am requesting that any fraudulent inquiries, collections, and accounts opened in my name be blocked and deleted in accordance with my rights under 15 USC 1681c-2 ( a ) of the Fair Credit Reporting Act ( FCRA ). Specifically, I request that Equifax block and delete : Collection Accounts : - XXXX XXXX XXXX -- - Balance {$13000.00} Inquiries : - XXXX XXXX - XXXX XXXX These inquires and collection accounts were created as a result of identity theft or unauthorized use of my personal information. I know this is a direct consequence of the data breach and should be promptly addressed. To support my request, I have enclosed the following documentation : A copy of my Identity Theft Report filed with the Federal Trade Commission ( FTC ). A copy of my government-issued identification, Proof of address Any other required documents for verifying my identity and the fraudulent activity. Please take the necessary steps to block and delete this fraudulent inquiries associated with identity theft from my credit file. In accordance with the FCRA, I request written confirmation that these actions have been completed and that my credit report has been updated accordingly.
