XXXX XXXX XXXX, am writing on my own as natural person and consumer who is a victim of identity theft. I am requesting the immediate removal of the following items from my consumer credit report in accordance with 15 U.S. Code 1681c-2. This statute requires that any information identified by the consumer as resulting from identity theft must be blocked from the consumer 's credit report within 4 business days of receipt of my request. It has been over 30 days since my initial request, and you are in violation of this law. This dispute is not being made in error, nor is it based on any misrepresentation of fact on my part. The information I am disputing is fraudulent, and the entities that reported these items DO NOT have lawful authority to do so. According to the law, these companies are required to have tax documents or other forms of written consent to charge off or report these items under my name. I did not provide such consent, and as such, these items are in violation of my privacy Under 15 U.S. Code 1681b, consumer reporting agencies may only furnish a consumer report in accordance with the written instructions of the consumer. I NEVER provided any written consent for any consumer reporting agency to report the information listed below on my behalf, and their failure to adhere to this law constitutes a clear violation of my rights as a federally protected consumer. Please delete the following items from my credit report immediately ADDRESSES INQUIRES NAMES ACCOUNTS COLLECTIONS These accounts should not appear on my consumer report, as they are the result of identity theft. Please ensure these fraudulent accounts are removed as required under the law. The companies reporting these items have failed to provide proof of my consent or authorization, which makes their actions unlawful. I respectfully demand that you take the necessary steps to delete these accounts from my credit report and send me written confirmation that these items have been removed. This request is made under the rights provided to me by the Fair Credit Reporting Act and related laws. Thank you for your immediate attention to this matter. I look forward to your timely response.
