I, XXXX XXXX XXXX XXXX XXXX, reviewed my credit report and found the following Name, Addresses, and inaccurate accounts reporting in my credit report. I am a victim of identity theft and did not authorize the charges. I have provided proof of identification, Proof of address ( Utility Bill ) and my social alone with the inaccurate names and accounts that needs to be removed. I reported the theft of my identity to the Federal Trade Commission, and I also have enclosed copies of the Federal Trade Commissions Identity Theft Affidavit. By the provision of the Fair Credit Reporting Act, I demand that these items be investigated and removed from my report. It is my understanding that's you will recheck these items with the creditor who has posted them. Please remove any information that the creditor can not verify. I understand that under 15 U.S.C Sec 1681 ( a ), you must complete this investigation within 30 days of receipt of this letter. Please send an update of my credit report to the above address. According to the act, there shall be no charge for this updated report. I also request that you please send notices of corrections to anyone who has received my credit report in the past six months
