I am writing to dispute fraudulent accounts that have been illegally opened in my name. This matter is of the utmost urgency, and I demand immediate correction of my credit report. The following accounts were fraudulently opened without my consent ( see attachment ). These accounts are fraudulent, and I did not authorize the creation of any of these accounts. I demand that these accounts be permanently removed from my credit report immediately. I reported the theft of my identity to the Federal Trade Commission and I also have enclosed copies of the Federal Trade Commission 's Identity Theft Affidavit. This situation has caused severe damage to my credit and has resulted in undue stress and Financial Loss. I expect prompt action in investigating and correcting this issue. I demand that the fraudulent accounts be removed and a new credit report be issued reflecting these changes. Please provide written confirmation that these fraudulent accounts have been deleted from my credit report. Failure to respond and correct this matter within 30 days will leave me no choice but to pursue all legal counsel to protect my rights.
