I, XXXX XXXX, under penalty of perjury, do hereby affirm and declare the following facts and truths regarding inaccuracies in my credit report as furnished by XXXX : In XXXX of XXXX I was informed that my personal information was leaked onto the internet. A company by the name of XXXX XXXX informed me that XX/XX/XXXXXXXX XXXX XXXX XXXX my information and my daughters were hacked and stolen. The information taken was my XXXX XXXX XXXX XXXX XXXX, Billing information and other personal information such as Social Security and driver 's license numbers. Upon reviewing my credit report, I found that I am the victim of identity theft. I have put a freeze on my credit and accounts with XXXXXXXX XXXX XXXX XXXX XXXX XXXX. I am requesting that the false information item ( s ) be blocked, corrected and removed from my credit report. Enclosed are copies of my credit report along with supporting documents to support my position. Please investigate and remove these items as soon as possible. Pursuant to the fair credit reporting act ( FCRA ) Section 605B ( 15 U.S.C 1681c-2 ) In the year XXXX XXXX XXXX my personal credit information was exposed in multiple data breaches EquifaxXXXX XXXX XXXX XXXX and XXXX XXXX who's letter to me is attached. My sensitive and private information was illegally removed from the list of companies and placed on the internet and dark web. These data breaches and the open disclosure of my private and sensitive information to the world has created a myriad of problems for me and most importantly my credit. You see I just opened and started a business and my credit is needed to build capital. XXXX XXXX XXXX XXXX XXXX XXXX and credit is sometimes needed to stay a float. Criminals and businesses are repeatedly using and running my credit affecting the score. These people have my full name, address, ss # and birthdays. These people have the ability with the information those companies had in my files the ability to freeze and unfreeze my credit if they wanted. I am constantly receiving unwanted emails and phone calls about loans I did not apply for. One is actually on my credit XXXX XXXX I was told that because nothing was actually stolen or gained I can't do anything. XXXX off doesn't actually give anyone money, they are a credit reporting agency. So the account was fully credited and loan approved because it's a pretend loan. You pay them to report to credit bureaus. Another major problem on my credit now is the bankruptcy because the data breaches caused issues there. Multiple accounts were left open after my collection charge off date. This company continued to collect funds after charge off and bankruptcy discharged because false applicants made them think the account was still open. That company is XXXX money if you look you will see that the account says closed XX/XX/XXXX but my bankruptcy is also listed. They kept taking funds directly from my paycheck {$130.00} every 2 weeks. I called and spoke with them and it took until XX/XX/XXXX for them to refund me and stop taking funds. The entire thing took from XX/XX/XXXX until XX/XX/XXXX because the bankruptcy deleted and closed my account. Very special higher up people had to work on the issue and refund over {$2000.00}. MXXXX XXXX XXXX XXXX are closed with multiple closed dates and super inaccurate information. One bad apple spoils everything and so I think the best and right thing to do is delete everything. XXXX showed guilt and proved me right by deleting items on my credit report immediately after getting this affidavit of dispute and various other information. In XX/XX/XXXX my credit score went from XXXX to XXXX on XXXX alone. The entire report should have been deleted but they started removing false and inaccurate information. Why do that unless the information is wrong and false. And they still have not shared which information they removed. So it's correct to assume I am right. So please remove everything from the reports especially when we know it is wrong information.
